CHAW Care Community
We’re excited to lead on an exciting new intuitive to create better
public services in our area. We can help you access health care by
signposting you to the service you need.
Cheshire East is made up of 8 Care Communities. The Guild represents
CHAW Community Care Team (Chelford, Handforth, Alderley & Wilmslow) as the
Voluntary, Community, Faith and Social Enterprise (VCFSE) sector lead.
All communities consist of health and social care teams to deliver
services that treat illness, promote wellness and self-care. Care
Communities include GP’s, Community nursing teams, Mental Health Teams, Social
Services and Voluntary, Community, Faith and Social Enterprise (VCFSE) sector
organisations.
Our overall aim is to:

and support initiatives
