Dementia Action Week.

Dementia can affect a person at any age but it’s more common in people over the age of 65.  Getting a diagnosis of dementia helps to determine the care plan that will be put in place to support you and your carer.

The Dementia Reablement Service works in partnership with other dementia support services like Alzheimer’s Society and Age UK Cheshire and Age UK Cheshire East.
